of when to use the Constituent Service Request Form:
- To request assistance with any
county agency such as help with a building permit or any other
county provided service;
- Reporting road hazards such
as high accident intersections, potholes, etc. on Cook County
owned and maintained intersecitons;
- Problems or issues with any County department;
- Lodging a complaint regarding any County service;
- Any other situation where you are requesting
our office to take some action to assist you with an issue.
